Home Care Through Family Care

Family Care is Wisconsin's managed long-term care program for eligible adults and seniors. Your MCO authorizes hours, and CVHC provides the care in your home.

What Is Family Care?

Family Care is Wisconsin's managed long-term care Medicaid waiver. Unlike IRIS — where the participant self-directs — Family Care delivers services through a Managed Care Organization (MCO). Your MCO assigns you a care manager (often an RN and a social worker) who assesses your needs, authorizes services, and coordinates providers.

In our service area, Family Care is administered through state-contracted MCOs that serve western Wisconsin. CVHC is a contracted provider with:

Both programs use the same Medicaid funding and cover the same kinds of services. The difference is who decides:

Family Care

Good for people who want:

  • A care manager to coordinate everything
  • Less paperwork on their own plate
  • The MCO to handle provider relationships
  • A more "hands-off" experience

IRIS

Good for people who want:

  • Maximum control over caregivers and schedules
  • Ability to hire specific people (including some family)
  • Flexibility to move budget across services
  • A hands-on, self-directed role

Your ADRC walks you through both options during options counseling before you enroll. You can switch between them during open enrollment periods.

How to Add CVHC as Your Family Care Provider

Already enrolled in Family Care?

Contact your MCO care manager and tell them you'd like CVHC as your supportive home care provider. They'll confirm our contract, authorize hours, and we'll coordinate the rest. Most transitions take 1–2 weeks.
