Family Care is Wisconsin's managed long-term care Medicaid waiver. Unlike IRIS — where the participant self-directs — Family Care delivers services through a Managed Care Organization (MCO). Your MCO assigns you a care manager (often an RN and a social worker) who assesses your needs, authorizes services, and coordinates providers.

Your ADRC walks you through both options during options counseling before you enroll. You can switch between them during open enrollment periods.
Contact your MCO care manager and tell them you'd like CVHC as your supportive home care provider. They'll confirm our contract, authorize hours, and we'll coordinate the rest. Most transitions take 1–2 weeks.
